Effective Date: February 01st, 2025

I understand that participating in indoor cycling classes and related activities at Cobalt Cycle involves risks of serious bodily injury, including permanent disability, paralysis, and death. Risks may be caused by my own actions, inactions, or negligence; the actions or inactions of others participating in the activities; the conditions in which the activities take place; or the negligence of the Released Parties named below.

I understand that indoor cycling is a strenuous physical activity and certify that I am in good health and physically able to participate in cycling classes and related activities. I have consulted with my physician regarding any medical conditions that may affect my ability to safely participate in these activities.

In consideration of being permitted to participate in activities at Cobalt Cycle, I agree to the following:

1. ASSUMPTION OF RISK: I knowingly and freely assume all risks, both known and unknown, associated with participating in activities at Cobalt Cycle, even if arising from the negligence of the Released Parties.

2. RELEASE AND WAIVER: I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless Cobalt Cycle, its officers, officials, agents, employees, other participants, and owners and lessors of premises (“Released Parties”) from any and all claims, demands, losses, and liability arising out of or related to any injury, disability, or death I may suffer, or loss or damage to person or property.

3. MEDICAL CONSENT: I understand that Cobalt Cycle does not provide medical insurance for participants. In the event of injury or medical emergency, I authorize Cobalt Cycle to secure appropriate medical treatment, if necessary.

4. HEALTH WARRANTY: I warrant that:
  – I have no medical condition that would prevent my safe participation
  – I am not pregnant (if applicable)
  – I am not under the influence of alcohol or any drugs
  – I have eaten appropriately and am sufficiently hydrated to participate

5. COMPLIANCE WITH RULES: I agree to comply with all rules and regulations of Cobalt Cycle, including:
  – Following all instructor directions and safety guidelines
  – Using equipment as instructed
  – Reporting any equipment malfunctions or safety concerns
  – Discontinuing participation and notifying staff if I feel unwell

6. PHOTO/VIDEO RELEASE: I grant Cobalt Cycle permission to use photos or videos of me taken during classes for promotional purposes without compensation.

By signing below, I acknowledge that I have read this waiver, fully understand its terms, and sign it freely and voluntarily without inducement.

This waiver shall remain valid from the date of signing unless explicitly revoked in writing.
